Registration

You can now register your student for the School-based Health Center online! The online form is available in English.

If you would like a form in a different language, please visit the Neighborcare website.

Support from the Families and Education Levy does not cover the entire cost of the Teen Health Center. If you have health insurance, we ask that you complete that section of the registration form so that we can bill your insurance company.

If you do not have health insurance, please indicate that you have no insurance under the insurance section of the registration form. Our Eligibility Specialist will then help your family find coverage.

For additional information about insurance, please call the Teen Health Center. No patient will be denied services due to inability to pay.

Health Center Services

Did you know that your child can get health care at the Roosevelt Teen Health Center? The Teen Health Center is located at Roosevelt High School and is operated by Neighborcare Health, a community health center with funding support from the City of Seattle’s Families and Education Levy.

The Teen Health Center is staffed full time by a Physician Assistant, Mental Health Therapists, and a Health Educator, providing all the services (and more) that your family doctor provides in a teen-friendly setting. These services include:

  • Evaluation and treatment of common health problems
  • Sports physicals and routine check-ups
  • Preventive health care
  • Reproductive health care
  • Health education
  • Immunizations
  • Individual and group therapy
  • Coordination of care with community health care providers
  • Community & school-based referrals
  • Anger management
  • Preventive education on tobacco, alcohol, and other drug use

Any student enrolled at Roosevelt High School can schedule an appointment to see our medical and mental health providers by having a parent or guardian complete the registration forms and return them to the school-based health center. Copies of the registration forms are available in the health center and online. Under state law, youth may independently access some health care confidentially without parental consent, including reproductive health and mental health services.

The Teen Health Center is open every day during and after school so your child can be seen at a time that is convenient for his or her schedule. The Teen Health Center operates on the school calendar.
